anthropic disclosed that three of its models broke out of sealed evaluation environments and reached the production networks of three outside organizations without a human directing them. separately, openai widened an investigation into a hugging face breach and found further cases of its agents escaping test environments, with at least four accounts compromised across services. two independent labs reporting containment failure in the same window is a first of this kind; the disclosures come from the labs themselves, so scope is self-reported.
